RI Trooper Log: Crank Calls, Forgery, Domestic Violence

WARWICK, RI — RI Trooper Log arrests July 29- Aug. 4 concerning Warwick involved charges of crank calling, a no contact order violation, and forgery.

Here are the basics on those RI State Police arrests:

No contact order violation On July 28, 2023, the ACI Investigations Unit charged Jonathan DeSimone, age 28, with last known address of 23 Manchester Street Apartment #23, West Warwick, Rhode Island, with the following: 1.) Violation of No Contact Order, 2nd offense. DeSimone was arraigned in Third District Court and held without bail.

 

Wanted on multiple charges On July 26, the Gaming Enforcement Unit arrested Rocky Phelps, age 40, last known address of 276 Main Street, Apt. 12, Marlborough, Massachusetts, on the following: 1) Third District Court Warrant for Failure to Appear for Arraignment on a Vandalism charge out of Lincoln Police Department; 2) Third District Court Warrant HWOB for a 46G Violation on a Larceny Under $1,500 charge out of Warwick Police Department; 3) Third District Court Warrant for Failure to Appear for a Technical Violation Hearing on a charge of Obtaining Property Under False Pretenses out of the Coventry Police Department; 4) Third District Court Warrant HWOB for Failure to Appear for a Technical Violation Hearing on a Shoplifting charge out of Warwick Police Department, and 5) Third District Court Warrant for Failure to Appear for a Technical Violation Hearing on a charge of Larceny Under $1,500 out of Cranston Police Department. Phelps was processed at the Lincoln Barracks and turned over to officials at Third District Court.

Crank/obscene caller On Saturday, July 29, at 1:10 p.m., troopers arrested Michelle Cuevas, 49, of 20 Jefferson Boulevard, Warwick, Rhode Island, on an affidavit and arrest warrant for the charge of Domestic- Crank or Obscene Telephone Calls originating out of the Rhode Island State Police Lincoln Woods Barracks. 

This arrest was the result of a barracks investigation and Cuevas being located by the Warwick Police Department. Cuevas was processed at the Lincoln Woods Barracks, arraigned, and released pending a future date at the Third Division District Court.

Domestic assault At 4:32 p.m., troopers arrested Shaquille Hazard, 29, of 23 Barry St., Providence, Rhode Island on an active Affidavit and Arrest Warrant for 1.) Domestic Assault, 2.) Domestic Disorderly and 3.) Assault originating out of the East Providence Police Department. 

The arrest was the result of a motor vehicle stop on Route 95 in the City of Warwick. Mr. Hazard was transported to the Wickford Barracks where he was processed and then transported to the East Providence Police Department.

Forgery At 11:21 am July 31, troopers arrested Shawn McCarthy, age 38, of 33 Woodhaven Road Pawtucket, RI on an Affidavit and Arrest Warrant for Forgery originating out of East Providence Police Department. This arrest was the result of a motor vehicle stop on Post Road in the City of Warwick. 

McCarthy was transported to Wickford Barracks where he was processed and then turned over to the East Providence Police Department.

DUI, Rte. 95 At 1:32 am Aug. 1, troopers arrested Taylor Hollinrake, 31, of 1422 Middle Road East Greenwich, Rhode Island for 1.) Driving Under the Influence of Intoxicating Liquor and/or Drugs -First Offense and 2.) Refusal to Submit to a Chemical Test -Second Offense. This arrest was the result of a motor vehicle stop on Rte. 95 in the City of Warwick.

 

Hollinrake was transported to Wickford Barracks where she was processed, arraigned before a Justice of the Peace and released into the custody of a responsible adult.
